Dropped from Akkapan Adan in Crypt of Decay.
I got one of these from the Named - "Devourer of All" In Crypt of Sul. So this box is either a random drop in Crypt of Sul or it's a random drop from any named in TBM. Either way, it's a random drop and not from one named only.
I had a feeling this was the case. Removed it from the nameds loot table and made a note about it. Thanks!
